Letta Code repeat analysis and trace audit
Judgment
Retain the new analysis as a bounded source-grounded account, not as a strictly better replacement for every old finding. Publication bytes match independently calculated hashes. The sampled committed/live memory, archive, skill-admission and mod-promotion findings are supported. The new account gains a consequential executable-adaptation pathway and more explicit recovery distinctions, while losing the old account's explicit hook ERROR-versus-BLOCK and Workflow security findings. No new substantive correction to the published new claims was established in this sample.
The revised omission policy is visibly adopted: report verification names consequential questions, follows alternative paths and records a stopping rationale. That is behavioral evidence, not proof of better detection or lower effort. New preparation-to-publication wall time is 64.38 minutes, versus 51.76 minutes old; summed analytical worker spans are 68.18 versus 54.58 minutes. Both have twelve analytical attempts and one repaired profile refusal, but their inspected and reported scope differs.
Successful publication conceals recoveries. All twelve new analytical result envelopes report exit zero, yet the embedded messages contain eight failed content checks, three of them masked by shell wrappers, nine nonzero search commands, one failed file read, and an interrupted provider response. The affected workers recovered. These findings support preserving detailed traces and enforcing existing command-status rules, not treating one repeat run as a causal test of the method revision.

Reporter identity comes from this audit's PI_PROVIDER, PI_MODEL and
PI_REASONING_LEVEL, independently of the audited launch profile. Both published
manifests declare pi-sol; new worker result envelopes and identity reports agree
on gpt-6.1-sol/medium. Nullable generic model/effort attempt fields do not
contradict populated worker_model/worker_effort fields.
This report follows Audit an analysis, the workshop commission and the work collection contract. The user restricted writing to this report, overriding the procedure's navigation-update step. No README, peer report, method, retained member, run state or source was edited. No agent was launched, target executed, network used, engine command invoked, integration attempted or commit made.
The old audit and coverage-policy comparison were hypotheses and navigation aids, not independent proof. This audit is not blind. Direct frozen-source inspection was expressly commissioned; no ingest, brief or ancillary report was created outside the sole write scope.

The original session was located by run-ID search, not inferred from a current

NR/state/versions/<sha256> preserves the content named by attempt outputs and
judgment pins. Worktree paths are locators, not run identities.
The audit independently checked publication and version bytes, read both syntheses and report-verification accounts, the new boundary and full profile, and selected records, historical profile repairs and source spans. It scanned all twelve original new analytical transcripts, including both repair-chain workers, for tool/model failures, command-status masking, read continuation, write scope and source-read authority. The inventory is 908 messages and 529 tool results: 408 reads, 73 shell calls, 33 writes and 15 edits. Diagnostic launch and later activity are excluded. Counts of tool results do not count the interrupted, unexecuted partial tool call as a successful write.
This is targeted static checking, not exhaustive reanalysis or certification of every record, quote, inventory, admission principal or source branch. The audit checked selected source semantics independently, but did not replay validators against audited state or execute the target. Provider/Cloud behavior, deployed isolation, complete recall/reflection selection, causal benefit and every source-to-record correspondence remain unknown. Old failure counts and repair windows are principally the old audit's measurements; old publication bytes, repair versions, selected source claims and report-verifier read counts were independently checked here. No private reasoning is reproduced.
Findings
R1 — Same source and worker profile; a real, limited method change
Both worktree HEADs match their preparation records and the method commits above. Both acquisition effects, boundaries and source HEADs name the same source revision; both source working trees are clean. New preparation records a clean origin. Old preparation recorded dirty-origin changes excluded, as the old audit reported; they are not part of the old method identity.
The commit-range diff contains three changed analysis-method files:
jobs-engine/verify-reports.md, jobs-engine/follow-worker-rules.md and
types/agentic-system-verification.md, all under
kb/agentic-system-analyses/. Other changes are workshop, report and instruction
material outside this analysis-method change. No analysis engine, schema,
role/dependency layout or analyst instruction changed in that range.
The new report-verification attempt actually pins the revised bytes, not merely a worktree with the new HEAD:

The change permits report-verifier searches beyond cited paths for consequential omission questions, calls for supported dispositions and effort-versus-breadth stopping, and asks for a brief retained account rather than a query ledger. Profile and synthesis verification remain record-based. Source, profile and method identities therefore permit a useful same-source comparison, but not a controlled experiment: analyst selection, generated records, service latency and output volume were not held fixed.
R2 — Publication and historical repair bytes agree
Independent stdlib SHA-256 calculations establish, for each run:
- All 12
effects/publish.json.newhashes match retained destination bytes, includingARTIFACT.yaml. - All 11 Markdown member hashes in the published manifest match their files.
- All 81 version-store files hash to their filenames.
- There are 32 completed attempts, twelve model and twenty code jobs, and 23 judgments: 22 accepted, one refused. Completion did not erase refusal.

These are local byte-correspondence checks, not independent provenance authentication or evidence that every claim is true. Publication is completed inside each isolated worktree; no main-checkout integration is implied.
R3 — Stable source-supported memory conclusions; selective gains and losses
| Question | Old/new retained account | Independent check and consequence |
|---|---|---|
| Committed projection versus current checkout | Old MEM-RTE-core-projection; new MEM-RTE-memory-projection; both MEM-RTE-deferred-read; both syntheses preserve the distinction. |
S/src/backend/local/system-prompt-compilation.ts:81–199 enumerates Git HEAD and reads HEAD:<path>. src/backend/local/local-backend.ts:896–976 preserves the cached original body on revision-only refresh while returning a newer memory update. src/tools/impl/memory.ts:306–405 reads current files. Neither publication warrants universal committed-only visibility. Memory-tool root-file rejection also prevents interpreting this as arbitrary root-content retrieval. |
Root ARCHIVE.md |
Both memory accounts and syntheses reject automatic archive-as-withdrawal. | src/agent/memory-format.ts:13–21 selects root Markdown in v2; compiler :150–199 excludes MEMORY.md from the ordinary loop, not ARCHIVE.md. New memory.md Write-side quotes both doctrine and selection. This is a predicate-based deduction, not an observed archival incident. |
| Hook ERROR versus BLOCK | Old EPI-RTE-hook-disposition and synthesis explicitly retain the distinction. New RT-RTE-hook-decision emphasizes eligible PermissionRequest asks; its synthesis does not preserve the old explicit PreToolUse error disposition. |
src/hooks/executor.ts:392–468 sets blocked for BLOCK, errored for ERROR and continues; src/tools/manager.ts:2192–2220 returns early for blocked, not merely errored. The old narrow finding remains supported. Its omission is a loss of useful coverage, not a contradictory new fail-closed claim. Do not transfer this result to every hook consumer. |
| Skill metadata admission | New profile repair separates source lookup from invocation admissibility. | src/tools/impl/skill.ts:273–288 rejects disable-model-invocation unless allowDisabledModelInvocation is set. The retained scoped validation/enforcement classification is supported; override availability limits rather than negates the ordinary gate. |
| Mod evaluation and executable promotion | New RT-RTE-mod-learning, EPI-RTE-marker-check, EPI-RTE-assertion-check and synthesis add a report/history-to-successor-to-conditional-promotion account. Old synthesis characterizes trusted mod mutation but leaves improvement links at mutability. |
src/mods/learning-harness.ts:2300–2375 supplies previous attempts and history to later candidates, selects a report and copies the selected source only when passing and an explicit promotion path is supplied. New positive promotion is not the old unsupported reflection-tier inference. This check supports the conditional copy/history chain, not every evaluator branch or actual activation/benefit. |
| Listener recovery | Both reject exactly-once effects. New RT-RTE-listener-recovery more explicitly separates provably unstarted calls from started calls with unknown outcomes. |
src/websocket/listener/recovered-approval-checkpoint.ts:45–168 records unknown before effects, restores unstarted state on pre-effect failure, and records exact results afterward. This supports the more precise state distinction. End-to-end durability and all recovery wiring were not reanalysed. |
The new functional boundary is whole-system; the old is complete artifact,
partial loop. Both are enclosing-runtime, code-grounded analyses of the same
repository with external Cloud/provider internals excluded. The new top-level
coverage table explicitly includes shipped entry and maintenance surfaces while
separating available-but-uninspected files from access gaps. Under the unchanged
boundary type, external providers do not automatically make the shipped harness
a subsystem; conversely, a whole-system label is not exhaustive inspection. The
old prose chose the partial-loop label because Cloud internals were unavailable.
This classification variation is not evidence that the new run acquired a more
complete source. Compare functional responsibilities and local limits, not the
label as a quality score. No new boundary blocker was established here.
Other coverage differs. New memory directly treats shared attachment/detachment, restore and lexical recall in more detail. Old runtime/synthesis retain a separate Workflow VM/non-security-boundary account and old epistemic/synthesis retain lexical secret-redaction behavior; these are not carried as equivalent new records. Those losses were identified from retained artifacts, not fully rechecked against source in this audit. New profile scope centers on MemFS, skills, transcripts and mods rather than reproducing the old changed hook/workflow/schedule inventory. Absence from the new profile is not evidence that those mechanisms disappeared from the unchanged source.
Both publications appropriately withhold observed learning quality, successful self-improvement and truth acceptance from persistent update wiring alone. The new executable-adaptation account is a significant gain, but these gains and losses are not attributable to the verifier revision: analysts already selected them before report verification.
R4 — Both profile refusals were repaired, for different reasons
The old repair was independently confirmed by version diff. Old attempt 000019
emitted reflection curation_operations: promote from generic tiering. Version
cc6252d99656b2ad74f0a83bf340141e84e3c7529392db7cf467a944b40c316a
was challenged by verifier version
6fb28aefdeb82bf7744669e44d9f421b7f2950d2fb4d334230133821e69545fc.
Attempt 000023 removed the seven-line positive and revised its note/rationale
because tiering did not establish upward direction. The published old profile
contains that repair.
New attempt 000019 instead produced
bc6a59bf0adfe2aca00a3fa73d4a293e91ed2b033a00c066c4f36084ffe03dba.
Its known selector-metadata unit emitted routing alone despite the records'
skill invocation gate. New verifier version
aa331a245b8f1e3352da18ceeafc291a08ec3d449f7240224665e683e43307b8
raised that mismatch. NR/state/judgments/000022-apply-profile-verification-1.json
records refusal, installs: false and no overrides. Attempt 000023 splits the
unit, adds wired validation/enforcement for the ordinary loading gate, and
updates the rationale. The direct version diff is local, not a broad rewrite.
Attempt 000025 accepts; judgment 000026 installs without overrides. Trace chain:
T L42, L46, L50, L54.
The new published promote value is specifically the passing-mod copy pathway,
not a recurrence of the removed reflection claim. Its source basis was checked
in R3. The new profile also keeps archive invalidation at claimed strength,
not compiler-enforced demotion. These are distinct parts and evidence strengths;
a system-level search for the string promote would produce a false regression.
Both repairs show a useful semantic safeguard against misleading extractable units. They do not establish that the revised report-verifier policy caused better profile review: the profile-verifier mission did not change, and its new objection concerns different evidence. An axis-wide partial assessment cannot make an unsupported positive or an incorrectly complete unit safe.
R5 — Omission questions and stopping are evidenced; efficiency is not
New report-verification.md#omission-search-and-stopping-decision selects three
families of questions:
- Could alternate callers invalidate admission/recovery conclusions?
- Could maintenance deliver uncommitted or differently selected content?
- Could alternate evaluator/integration branches support stronger acceptance or improvement conclusions?
The trace supports work behind the prose. T L36/R0/m76 enumerates the frozen
top-level tree and searches reflection callers, compaction handoffs and effect
callbacks. Reads at m78, m98–100 inspect explicit reflection integration and
transcript selection; m101–102 inspect distinct sandbox fallback boundaries;
m106–111 inspect headless continuation and provider seams. Earlier reads
m50, m63–65, m69–72 address committed projection, live retrieval, retained
summaries, skills and candidate promotion. These are actual source reads, not
only a restated author checklist.
The three questions receive scoped dispositions in the verification account.
Explicit integration requests review of accuracy, placement, precision and
consistency (S/src/cli/helpers/reflection-integration.ts:11–38), but does not
establish an independent truth oracle. The verifier retains partial front-door
and reflection-selection/evaluator coverage as limits. It stops because its
selected challenges are disposed, no known contradiction is left undisposed,
and the remaining bounded claims do not require exhaustive safety, absence or
benefit conclusions. The report does not quantify expected marginal search
value, but the revised rule did not require a numerical budget. Its stopping
rationale connects retained breadth to the cost of further optional inspection.
The dedicated omission section is 191 whitespace-delimited words. The full new report-verification member is 1,415 words versus 1,882 old, using the same simple split count including frontmatter, links and tables. The old member included a separate ten-row source-read log; the new uses source anchors in its claim checks rather than that ledger. Thus the explicit account did not make this member longer. It did not make verification faster: direct source-read calls increased from 18 to 41, all tool calls from 51 to 82, and worker span from 267.205 to 449.432 seconds. Read calls are neither unique coverage nor tokens. The new reports also supplied more memory/epistemic material and a new mod-learning chain to verify.
The old verifier already independently checked committed/live visibility, archive behavior, hook errors and optional reflection review. Repeated checks of the retained conclusions are not new detection attributable to question-driven checking; the explicit hook-error distinction was not retained in the new account. The new report verifier found no blocker; the later profile verifier found and repaired a classification problem. With no controlled question-selection or claim-breadth comparison, neither superiority nor inefficiency caused by the revision follows from this pair.
R6 — Recovered tool failures and masked shell status
All eight failed new content checks are accounted for:
| Worker | Failure results and final recovery, T anchors |
Status behavior |
|---|---|---|
| Boundary | L20/R0/m60: attribution syntax; parser read and URL repair; pass m68. |
Failure propagated. |
| Runtime | L24/R0/m82: attribution syntax; m86: nonexistent quotation of PermissionRequest arguments; source reread m88, corrected arguments, later pass m98. |
Both failures propagated. The missing quote was not accepted as source. |
| Memory | L28/R0/m102: attribution syntax; m106: repeated headless skill-injection quotation; supplied range and additional source/quote checks; pass m122. |
Both print exit=1 but return isError: false. |
| Epistemic | L28/R1/m71: attribution syntax; m75: duplicated compaction quote; range repair; pass m79. |
Both failures propagated. |
| Report verification | L36/R0/m117: reflection-integration attribution syntax; output reread and pinned URL repair; pass m123. |
Prints exit=1 but returns isError: false. |
The three masked validator failures use validate ...; printf ... "$?" without
returning the saved status. The final successful printf determines shell
success. Workers read the diagnostics and repaired them, so no demonstrated
acceptance bypass occurred. The old audit counted seven failed validators and
five masked validator failures. This is recurrence with successful recovery,
not an estimated failure rate or a causal improvement from five to three.
There are also nine nonzero search calls: Runtime
L24/R0/m19,m21,m30,m38,m40,m55, and Epistemic
L28/R1/m33,m42,m50. Eight report guessed missing files; Runtime m40 reports
exit 1 without a missing-file diagnostic. All nine correctly propagate failure.
Memory L28/R0/m90 separately reports ENOENT for guessed
src/agent/memory-frontmatter.ts. Subsequent work uses the actual root
src/memory-frontmatter.ts. Missing guessed paths were not published as proof
of absent mechanisms.
Pipeline discipline remains uneven. Runtime L24/R0/m23,m48,m63 and Epistemic
L28/R1/m52 use head/tail pipelines without pipefail; their displayed
zero is not proof that every upstream command succeeded. Memory begins adding
set -o pipefail at L28/R0/m57, but semicolon-separated commands and a final
printf still fail to preserve each earlier status. This is a demonstrated
rule-compliance risk, not an invented count of hidden SIGPIPE or failed
searches: the retained outputs do not expose every pipeline component status.
The positive counterexamples are the explicit status propagation in Boundary,
Runtime's failed searches/checks, Epistemic checks and later profile/synthesis
checks.
Memory's scratch/check-repairs.md survives inside T L28/R0/m119–120: it
records quote replacements, the supplied range, and added archive/visibility
support, explicitly saying no finding was narrowed to pass validation. Runtime's
quote correction replaces wrong call arguments with the actual inspected call
(T L24/R0/m87–90). No unsupported finding deletion merely to silence a
validator was established in the inspected repair sequences.
R7 — A successful worker envelope contains a provider failure
T L46/R0 reports exitCode: 0 and final stopReason: stop, but its
errorMessage says “OpenAI Responses stream ended before a terminal response
event.” Embedded m52 is an assistant response with stopReason: error and
durationMs: 28159. It contains a partial write request for profile verification,
not a completed tool result. The next assistant message, m53, supplies a full
replacement write; m54 is the successful write result. Identity/answers and
validation follow, with a pass at m59 and final completion at m60.
This is a recovered transport/provider-stream failure inside a model attempt,
not an extra engine attempt or a missing published report. The historical
verifier bytes, profile refusal and subsequent repair chain remain intact.
The reported failed-response duration is 28.159 seconds, included in the
worker span; this audit cannot isolate its net latency, retry policy or billing
cost. The same scan of the old twelve analytical transcripts found no embedded
assistant stopReason: error. One occurrence does not establish a regression
caused by the method revision.
R8 — Read recovery and authority were preserved in the inspected actions
Boundary's large tracked-file listing was genuinely outer-truncated at
T L20/R0/m15: only lines 1102–2417 were delivered initially. It recovered the
missing prefix with /tmp/pi-bash-f7d6a6df82438c32.log reads at m17 (1–400),
m19 (401–800) and m21 (801–1101). This is complete recovery, not an ignored
truncation. Multi-range supplied runtime, memory, epistemic and profile reads
continue through their tails across the downstream workers. The apparent final
blank-line continuations are not missing substantive text. Strings such as
[Output truncated...] appearing inside inspected target source are not harness
truncation notices. No unrecovered mandatory-input truncation was established.
The write/edit inventory and shell commands keep writes to assigned outputs, identity reports and scratch. The profile generator runs in its own scratch and writes its assigned output; the repair copies its supplied previous profile to its own output. No source execution, prior-review exposure, cross-worker write, state mutation or worker delegation was found among analytical tool actions. After analysts, only report verification reads target source; reconciliation, profile/repair/verifiers and synthesis roles remain record-based in these traces. The report verifier's broader searches are expressly allowed by the new rule, not an authority expansion to all downstream roles.
A suspected path deviation did not survive checking the actual handouts.
The shared rule says to use supplied absolute paths, but these invocations
supply worktree-relative output and identity paths. The full prompt results at
T L20/R0/m3, L24/R0/m3 and L50/R0/m3 show that runtime output
L24/R0/m80 and repair output L50/R0/m53 use the supplied paths rather than
inventing shortened ones. This is a wording/delivery mismatch, not established
worker disobedience or a write-authority escape. No misdirected read/write was
found. Any future path-contract alignment should inspect the producer as well
as the instruction before assigning blame.
The coordinator reads publish-analysis.md at T L37–38, before publication's
advance/result at L63–64. Like the old run, this is a counterexample to a late
publication-prerequisite read. No claim is made that every coordinator rule was
exhaustively audited.
Timing and repair costs
Worker spans are the minimum-to-maximum numeric message timestamps inside each embedded transcript. They include model/tool waiting within that interval, not just model compute or full launch overhead. Old values below are the old audit's same measure. Both runs overlap Memory and Epistemic; other analytical launch groups are sequential.

| Summed worker spans | 4,090.763 (68.18 min) | 3,274.903 (54.58 min) |
New preparation-call through publication-result is 3,862.673 seconds,
T L13–64, 11:37:11.191Z–12:41:33.864Z. Start-call through that result is
3,853.815 seconds. Old preparation-to-publication is 3,105.776 seconds. The
increase is 756.897 wall seconds (24.4%), while summed worker time increases
815.860 seconds (24.9%). Summed worker time exceeds wall time because of the
parallel lens pair; neither quantity is billed compute. Later inspection,
audit time and diagnostic launch are excluded.
The new profile repair plus fresh re-verification costs 361.794 summed worker
seconds, versus 305.940 old. Its launch-to-return window is 384.711 wall
seconds, T L49–54, versus 330.386 old. Initial verification is useful
detection work, not all repair waste. Both runs have the same number of repair
attempts; extra attempts do not explain this repeat's longer duration.
Local failed-check-to-final-pass intervals are Boundary 29.880 s, Runtime 93.811 s, Memory 138.732 s, Epistemic 63.925 s and report verification 21.150 s (the R6 endpoints). Their sum is 347.498 worker-span seconds, already inside the above totals. These windows include further evidence work and edits, overlap between the lenses, and exclude pre-failure drafting; they are not isolated quotation-repair cost and must not be added to total wall time. The 28.159 s interrupted response is likewise already included.
Harness totals for new analytical workers are 355 model turns and 5.1998064 reported cost units, versus old 317 and 4.5389664. These exclude the coordinator, diagnostic and auditor, and are not an invoice. Most added summed time is in Memory (+265.696 s), report verification (+182.227 s) and Epistemic (+127.914 s). New memory, epistemic and profile members are longer; runtime and report verification are shorter. Worker selection, source exploration, report breadth, repairs, caches and service latency remain confounds. Same source and same overlap pattern improve comparability, but do not isolate a method effect.
Dispositions and recommendations
Local result and correction decisions
- No replacement analysis is justified by this sample. Retain the new bounded positives and explicit unknowns. A contradictory source-supported emitted claim would change that disposition. Byte checks do not substitute for that semantic decision.
- Do not discard old findings on the assumption that the new set subsumes them. The old narrow PreToolUse ERROR/BLOCK finding remains source-supported; Workflow and secret-redaction coverage also differ. A separately authorized correction or comparison could retain consequential lost findings. Benefit: preserve consumer-relevant conditions. Cost: additional record ownership and review, not silent merging of frozen sets. Evidence against doing it would be a consumer question for which those differences are immaterial.
- Treat boundary-kind variation as a comparison caveat. No missing source was acquired between runs. Normalize the intended comparison responsibility before using whole-system/partial-loop labels to select or rank results; do not silently rewrite either frozen boundary.
Method findings, not adopted changes
- Question-driven omission checking was exercised with a brief account. The traces and dispositions support adoption, not a measured quality or efficiency benefit. Retain the revised policy provisionally rather than expand it on this pair. A separately authorized bounded comparison would need comparable supplied claims, actual search effort and independently checked consequential omissions; finding count alone would not discriminate quality.
- Apply existing shell-status rules. Masked validators recur despite
successful textual recovery;
pipefailalone does not preserve semicolon-separated commands. Status-preserving construction has low added cost and makes failures visible. No additional policy paragraph is justified merely by repetition of an existing-rule violation. The separate relative-path observation is a handout/rule wording mismatch, not a proven worker violation. - Preserve embedded traces and historical refusals. The recovered provider failure would be missed by checking only exit zero and final completion. Retention/audit effort is the cost; identifying a meaningful trust or recovery difference is the benefit. This is a local recovered service event, not a demonstrated method defect or evidence for another reviewer role.
- Preserve semantic profile review. Both runs repair misleading extractable classifications, for different mechanisms. The new repair's added cost is measured above. A partial axis is not a substitute for correct unit values.
Multiple repairs, questions or reviewers inside one run are not independent
runs. These observations do not establish a detection rate, a general method
cause or an instruction-change mandate. Broader graph discovery through
cp-skill-connect is a possible separately authorized next step, not work
performed under this sole-report commission.
Verification and preservation
Read-only stdlib calculations and Git inspection were used for evidence checks; no analysis-engine inspection or advance command was run by this auditor. Only this report was authored. The pre-existing disjoint Napkin repeat report was left untouched, as were any other workers' report paths. Workshop navigation was intentionally not updated under the user's explicit restriction.
