EQUIPA memory report
Type: agentic-system-analyses/types/agentic-system-memory-report.md
Boundary and evidence
The subject is EQUIPA at the Git revision in boundary.md. This member includes retained task experience, project context, initiative plans, role-instruction evolution, recovery snapshots, inter-agent messages and working-context controls. It does not treat SQLite storage, model calls or a feature name as evidence of learning effectiveness. Source-native names such as Reflexion and MemRL identify programs, not demonstrated properties.
All direct inspection below is SRC-1 implementation evidence, except the interactive skills, which are doctrine/design. HEAD was rechecked against the supplied full revision and git status --short returned no changes. The supplied runtime contributes the separately attributed routes below. No target, model, test or service was executed. External model weights and host compaction are opaque; operator databases, deployed configuration, actual stored content and independent execution evidence are unavailable. These gaps prevent conclusions about activation, retrieval quality, complete recovery, safety or improvement.
Coverage inventory
“Uninspected” below means available source not examined deeply, not inaccessible evidence. The inventory preserves these included parts without attributing their unknown internals to better-inspected paths.

RT-RTE-forgesmith-changes and related supplied evolution records cover known read-back. Rubric scorer, OPRO, complete evaluation/rollback semantics, training-data lineage/weight-training/deployment and sweep validity remain uninspected. Cannot infer weight learning or complete optimization coverage from tool names. |
| Housekeeping and other file/database maintenance | Search of .claude/skills/housekeeping/SKILL.md finds instruction-driven completed-plan deletion; setup/migration/sync covered only by boundary inventory |
No enforced live cleanup or database migration/admission correctness established. These remain included but do not inherit SIMBA/session cleanup semantics. |
Core ideas
- Experience text and experience access policy are different retained results. Episodes store agent-produced summaries/reflections; outcomes and Q-values influence host selection. Outcome-wide rewards do not establish that an episode helped. Embeddings and graph scores are selection inputs, not text reasons shown to the task agent. See MEM-RTE-episode-feedback and MEM-RTE-experience-retrieval.
- Admission is route-specific. Regular lessons are template-derived from repeated error signatures; SIMBA inserts model rules under structural/duplicate checks; episode prose is reject-sanitized; project notes may be instruction-driven human/agent entries. Read-time sanitization does not mean all writers share a write gate. See MEM-RTE-lesson-maintenance and MEM-RTE-project-context.
- Recovery is reconstruction, not conversation restoration. Hard checkpoints retain output; soft checkpoints retain paths and output prefixes; sessions aggregate these with state-file hints and a short tool trace. Later prompts receive selected, sanitized text, not the original engine frame. Their delivery is additionally gated by history selection. See MEM-RTE-context-recovery.
- Withdrawal at one consumer need not remove access elsewhere. Inactive SIMBA lessons leave automatic injection but remain queryable by the inspected MCP lesson handler. A Q-value below the threshold excludes an episode from that retrieval without deleting it. GEPA rollback selects baseline while retaining version files. These are distinct operations, not universal forgetting. See MEM-RTE-memory-queries and RT-RTE-gepa-prompts.

Components
MEM-CMP-embedding-service
Label: Embedding service
Source-native identity: configurable Ollama embedding model/base URL used by get_embedding; default all-MiniLM-L6-v2. Representational form: distributed-parametric inference; service weights are external, while returned vectors are retained separately. Storage substrate: external Ollama model/service. Parameters changing during operation: uninspected — this caller requests inference, not training. Exact version pinned: not established by a configurable model name. Mutable endpoint resolving to another model: possible through model/base URL configuration. This differs from the chat endpoint in RT-CMP-ollama: different operation and consumer, not an independently proven different deployed service. SRC-1, equipa/embeddings.py.

MEM-OBJ-agent-episodes
Label: Agent episodes
Source-native identity: agent_episodes approach/reflection fields with task/role/project lineage and outcome/error metadata. Representational form: natural-language approach/reflection, symbolic outcome and identifiers; dense embedding and Q-value selection are separately assessed below. Storage substrate: SQLite. Raw task output is parsed into derived prose fields, not retained here as a full trace. A standalone model may replace a null reflection using only the output tail. Later task agents see shortened prose; SIMBA sees episode fields for rule generation. These are historical guidance, not privileged executable commands. Closest supplied record is RT-RTE-dev-test, a producer/consumer loop rather than this cross-task artifact; no supplied object counterpart. SRC-1, equipa/lessons.py, equipa/reflexion.py.
reflection = sanitize_episode_text(parse_reflection(result_text), "reflection") ---
equipa/lessons.py
MEM-OBJ-q-value
Label: Q value
Source-native identity: agent_episodes.q_value, plus process-local _injected_episodes_by_task and durable times_injected access telemetry. Representational form: symbolic numerical metadata interpreted by fixed rules, not distributed model weights or prose evidence. Storage substrate: SQLite plus process memory. Outcome-derived updates shape later ranking and threshold eligibility, not reflection wording. Tracking by task ID holds the latest assigned list, not a complete per-invocation exposure history. Distinct from episode content and from supplied RT-RTE-dev-test. SRC-1, equipa/lessons.py, equipa/prompts.py.

equipa/prompts.py
MEM-OBJ-lessons-learned
Label: Lessons learned
Source-native identity: lessons_learned text, error signatures, source/role, active state, counts and effectiveness score. Representational form: natural-language rule text and symbolic admission/access metadata. Storage substrate: SQLite; embedding is separately assessed. Regular ForgeSmith templates and SIMBA-generated rules have different write control despite sharing this table. Later consumers are task prompt construction, active-rule episode scoring and explicit MCP queries. SIMBA extracts rationale from candidates but the inspected INSERT omits it: future task injection conveys rule/error text, not that candidate rationale. No counterpart among supplied runtime objects; its nearest overlap, RT-RTE-forgesmith-changes, changes config/prompt files rather than this table. SRC-1, forgesmith.py, scripts/forgesmith_simba.py, equipa/lessons.py.

MEM-OBJ-embedding
Label: Embedding
Source-native identity: JSON float vectors in lessons_learned.embedding and agent_episodes.embedding. Representational form: distributed numerical representation, serialized in symbolic JSON; not readable reasons or model weight updates. Storage substrate: SQLite. Lineage is embedding inference on lesson text or sanitized approach/reflection; configured model version and historical vector compatibility are unresolved. Consumer computes cosine similarity; task agent receives associated text, not vectors. No supplied object counterpart; produced using MEM-CMP-embedding-service. SRC-1, equipa/embeddings.py, equipa/lessons.py.
"UPDATE agent_episodes SET embedding = ? WHERE id = ?", (json.dumps(embedding), episode_id), ---
equipa/embeddings.py
MEM-OBJ-lesson-graph-edges
Label: Lesson graph edges
Source-native identity: lesson_graph_edges rows with src/dst integer IDs, edge type and weight. Representational form: symbolic graph/access metadata. Storage substrate: SQLite. Lesson embeddings generate similarity edges; episode co-injection also supplies IDs to co-access edge creation. Consumer loads one adjacency and reranks episode candidates using graph PageRank. This is not necessarily a graph of formulated claims: the inspected schema has no node-kind column, while lessons and episodes have separate integer identity spaces. Mixed-producer identity warrants caution. No supplied counterpart; distinct from text and vectors. SRC-1, equipa/graph.py, equipa/lessons.py, equipa/prompts.py, schema.sql.
UNIQUE(src_id, dst_id, edge_type) ---
schema.sql
MEM-OBJ-checkpoints
Label: Checkpoints
Source-native identity: task/role/attempt .txt files storing agent output. Representational form: natural-language raw output, potentially containing structured markers. Storage substrate: checkpoint directory on disk. Timeout/max-turn continuation writes these files; later load compacts selected sections to 200 words and wraps them as untrusted historical output. This is distinct from the task branch in RT-OBJ-task-branch and supplies recovery to RT-RTE-dev-test. Reasons survive only if output and compaction retain them; no full conversation survives by virtue of this object. SRC-1, equipa/checkpoints.py, equipa/prompts.py.
filepath.write_text(output_text, encoding="utf-8") ---
equipa/checkpoints.py
MEM-OBJ-soft-checkpoints
Label: Soft checkpoints
Source-native identity: task/role/turn JSON files holding files read/changed, last output prefix, turn/compaction counts and signals. Representational form: symbolic state envelope and natural-language output; paths are metadata, not file contents. Storage substrate: filesystem. Different from hard output checkpoints: automatically sampled streaming state and different recovery rendering. No supplied object counterpart; supports RT-RTE-dev-test. SRC-1, equipa/checkpoints.py.
truncated_text = last_result_text[:SOFT_CHECKPOINT_TEXT_LIMIT] ---
equipa/checkpoints.py
MEM-OBJ-agent-sessions
Label: Agent sessions
Source-native identity: agent_sessions.state_json rows keyed by task, role, project and cycle. Representational form: symbolic state, natural-language partial output and optional state-file hints; the name partial_reasoning does not prove access to hidden model reasoning. Storage substrate: SQLite. Derived from soft checkpoint, .forge-state.json and up to 20 recent tool calls. Formatter displays tool name/turn/ok, paths and selected output/state hints, not full tool payloads or file contents. Different from both checkpoint tiers and from provider sessions. No supplied object counterpart; recovery consumer is RT-RTE-dev-test. SRC-1, equipa/sessions.py, equipa/checkpoints.py.

equipa/sessions.py
MEM-OBJ-project-context
Label: Project context
Source-native identity: session_notes summary/next steps, open_questions question/context and decisions decision/rationale/date. Representational form: natural-language history, questions and judgments with symbolic project/status/time metadata. Storage substrate: SQLite. Original content may be human-supplied or agent-written; generic DB access does not establish authorship or per-item human approval. Latest note, unresolved questions and five recent decisions reach task prompts; these are not task-agent conversations. No supplied object counterpart; nearest supplied consumer RT-RTE-dev-test. SRC-1, equipa/tasks.py, equipa/prompts.py, .claude/skills/forge-end/SKILL.md.

equipa/tasks.py
MEM-OBJ-initiative-plan
Label: Initiative plan
Source-native identity: .equipa/initiative-<id>.md containing initial plan and appended subtask outcomes, summaries and decisions/rationales. Representational form: natural-language working memory with symbolic Markdown markers and task/status fields. Storage substrate: project file/Git. Orchestrator parses agent output and writes sanitized/capped entries; operator plan editing is an additional declared surface with governance uninspected. Later sibling task prompt receives plan as historical data. Different from the branch container in RT-OBJ-task-branch: a particular cross-task context artifact, not equivalent to all product state. SRC-1, equipa/initiative.py, equipa/dispatch.py.

MEM-OBJ-agent-messages
Label: Agent messages
Source-native identity: agent_messages task/cycle/from/to/type/content/read-state rows. Representational form: natural-language or structured serialized messages and symbolic routing/access metadata. Storage substrate: SQLite. Raw tester failure details or other agent text are stored; selected sanitized content crosses into a later role invocation. Reading is not proof of response or useful collaboration. Distinct from RT-RTE-dev-test's complete loop. SRC-1, equipa/messages.py, equipa/loops.py.

equipa/messages.py
MEM-OBJ-persisted-output
Label: Persisted output
Source-native identity: optional large-output text files and returned preview/path message in tool_result_storage. Representational form: original text payload versus natural-language preview plus symbolic file path. Storage substrate: filesystem and returned string. Write-once mode prevents rewriting an existing filename; the newly computed preview can still reflect the newly supplied content, so preview is not independent authentication of file bytes. No supplied counterpart; distinct from checkpoints despite both retaining output. A later consumer/reload is not established by this helper alone. MEM-RTE-episode-feedback
Label: Episode feedback
Endpoints: completed task/result/outcome → parsed episode INSERT → optional standalone reflection update → exposure-linked Q-value update → later retrieval. Owner: CLI telemetry, episode code and fallback model. Distinct from RT-RTE-dev-test: this is cross-task experience production and metadata update, not the execution loop. SRC-1, equipa/cli.py, equipa/lessons.py, equipa/reflexion.py.
- implementation conclusion status: wired
- operation conclusion status: uninspected
- Immediate return: telemetry returns no task-agent answer; episode row and optional vector/reflection are retained if writes succeed.
- Later read-back: later system-prompt construction selects episode text using Q-value and other relevance inputs; SIMBA can read episodes too.
- Delegated visibility: fallback reflection model sees title, role, outcome, turns and last 1500 output characters; it does not see the complete task conversation.
- Selection predicate: post-task telemetry records outcome; fallback runs when raw parse finds no reflection; update reward applies to the process-local list popped for task ID.
- Invalidation or expiry: rejected text becomes null or is withheld at retrieval; threshold can remove future eligibility without deleting row; tracker vanishes on process exit and is popped after update.
- Activation or effect: program updates durable prose/access state; no observed task behavior or demonstrated capacity gain.
- Evidence limits: whole-task success rewards every tracked episode without individual-use attribution; tracker is latest-list per task rather than a durable exposure ledger.
Admission: agent output proposes approach/reflection; code parses and reject-sanitizes it. Fallback model text must have successful return, sufficient length and pass sanitizer before replacing a null reflection in the newest matching task episode. No per-reflection human approval or correctness oracle is shown. Model interpretation of a 1500-character tail is not a reference answer. Operating mode is post-task reflection on open work. Initial Q-value is outcome-derived; later successful outcome (tests_passed or no_tests) adds 0.1, otherwise subtracts 0.05, clamped to [0,1]. This is trace-fed access-policy modification, not a demonstrated reinforcement-learning weight update. Recovery is best-effort logging, not transactional restoration of every telemetry step.
The direct CLI call to maybe_run_reflexion omits dispatch_config, as does that function's call to record_agent_episode; therefore optional episode embedding in that recorder is a conditional capability but is not established as enabled by this ordinary telemetry chain. This qualifies any inference from the embedding code's presence.

MEM-RTE-lesson-maintenance
Label: Lesson maintenance
Endpoints: failed run signatures → template lessons; alternatively mixed-outcome episodes → SIMBA model rules → validated INSERT → effectiveness update/pruning → active lesson retrieval. Owner: ForgeSmith/SIMBA. Different from file/config changes in RT-RTE-forgesmith-changes. SRC-1, forgesmith.py, scripts/forgesmith_simba.py.
- implementation conclusion status: wired
- operation conclusion status: uninspected
- Immediate return: count of lessons added or stored/generated/pruned rule descriptors; persistence in
lessons_learned. - Later read-back: active lessons enter automatic role/error-type prompt retrieval; active SIMBA candidates are also reread for duplicate avoidance; explicit MCP lesson queries have a different active policy.
- Delegated visibility: SIMBA inputs include role-grouped success/failure episodes with reflection and hard cases, plus existing rules; later task agent receives selected rule/error text, not the whole synthesis input or discarded rationale.
- Selection predicate: regular extraction needs at least two recurring failed signatures; SIMBA selects recent reflected episodes with mixed role outcomes and low-Q early-termination cases; configured pipeline invocation and dry-run constrain writes.
- Invalidation or expiry: SIMBA prunes active
simba_generatedrows when injection count exceeds threshold and effectiveness is null or nonpositive; sets active to zero, not deletion. Regular active filtering and read-time sanitization separately affect visibility. - Activation or effect: durable text and eligibility changes can shape later prompt selection; generation quality, real usage and improvement unobserved.
- Evidence limits: before/after role success rates are confounded proxies, not causal rule effects; uninspected model internals cannot establish reliable criticism or valid rule induction.
Regular extraction sanitizes signatures, increments count for existing matches or derives a template by error class, validates structure and sanitizes text. SIMBA model proposes up to the configured per-role cap; software checks type, 20–250 character length, allowed failure class and greater-than-60% word-overlap duplicates before storage. The inspected SIMBA storage gate does not call the episode/lesson sanitizer; later automatic injection sanitizes. The variable rationale is read but not inserted. This separates authorship, software admission and actual file/database I/O. Neither route requires per-rule human acceptance. Existing rule rows and future manual database changes are not evidence of human review. Operating mode is trace-driven maintenance; episode outcome labels are proxy comparisons, not an expected-correct-rule oracle. Pruning is recoverable by database policy/editing, but no dedicated automatic reactivation procedure is established.
SIMBA evaluation uses at least ten injections and at least three episodes on each side of creation; the before window is thirty days. It persists the after-minus-before success rate, then pruning may withdraw automatic delivery. This update is trace-fed curation even though improvement attribution is not supported.

MEM-RTE-experience-retrieval
Label: Experience retrieval
Endpoints: task/role/project/error and feature flags → lesson/episode queries → ranking/deduplication/rejection/truncation → dynamic prompt suffix → Claude/Ollama consumer. Owner: prompt construction and selector code. Distinct from RT-RTE-dev-test's surrounding iteration and RT-BAP-role-instructions's static instruction authority. SRC-1, equipa/prompts.py, equipa/lessons.py, forgesmith.py, equipa/embeddings.py.
- implementation conclusion status: wired
- operation conclusion status: uninspected
- Immediate return: assembled PromptResult with selected experience text and exposure telemetry updates.
- Later read-back: prior task material reaches new inference calls automatically during prompt construction, without an agent asking for particular episode IDs.
- Delegated visibility: model sees shortened summary/reflection or lesson/error text, not all stored rows, vector coordinates, relevance scores or synthesis rationale.
- Selection predicate: feature flags; lessons match active role-or-global and optional error type, ordered by frequency/recency, fetched ten then deduplicated/capped five; episodes match role/project, Q > 0.3 and nonempty reflection, then relevance ranking and limit two or three.
- Invalidation or expiry: sanitization can discard an episode or lesson; Q/active changes alter future eligibility; static prefix above estimated 6000 tokens reduces episode count; final suffix trimming can further omit text.
- Activation or effect: delivery and host ranking are wired; actual model reliance, compliance and benefit remain unobserved.
- Evidence limits: injection counters and Q tracking occur before final prompt trimming, so they do not authenticate final full-text exposure; heuristic ranking is not demonstrated relevance.
Primary episode query fetches max(limit*3,10) candidates. Explicit literal-True episodes_cross_project allows role/task-type fallback; default is same project. Recency doubles scores within seven days; keyword overlap boosts them; vector similarity can blend 60/40; a rule-keyword/outcome bonus and optional graph reranking can intervene. Vector search scans stored vectors globally but only scores candidates already admitted by the episode query. Thus enabling vectors does not itself bypass project filtering. Q, counters and graph metadata have executable authority over selection; delivered text has advisory historical authority at model inference.
Counterexamples to idealized retrieval matter. get_active_simba_rules selects source simba or forgesmith, whereas inspected SIMBA writes simba_generated: those new rules can enter general lesson injection but do not match this source-based episode scoring bonus. With knowledge_graph enabled but empty adjacency or a caught reranking error, the code's final relevance sort is skipped because it tests the flag rather than reranking success. Consequently the fallback need not be keyword-score order. Graph mixing of lesson-similarity IDs with episode-coaccess IDs remains unresolved, not a proven coherent semantic network.

MEM-RTE-context-recovery
Label: Context recovery
Endpoints: streaming output/state or completed role call → checkpoint/session or derived summary → restored/consolidated extra context → next developer/model invocation. Owner: streaming code, loop, formatters and feature flags. This is the recovery/context-management part of RT-RTE-dev-test, not a replacement for its task-control account.
Part of: RT-RTE-dev-test
- implementation conclusion status: wired
- operation conclusion status: uninspected
- Immediate return: checkpoint path, session row ID, compacted summary or recovery prompt; loop continuation consumes the generated text.
- Later read-back: later role/task invocation can receive hard-output compaction, soft-state recovery or nonexpired session reconstruction; same-run summaries cross fresh inference frames.
- Delegated visibility: selected output sections, paths, tool names/status and state hints survive; full prior conversations, tool payloads and file contents do not enter through these formatters.
- Selection predicate: timeout/max turns; streaming periodic or suspected-compaction checkpoint; session_persistence flag on restore/capture; task/role selectors and anti_compaction_state history gate.
- Invalidation or expiry: accepted outcomes clear checkpoints; session restore filters fourteen-day expiry and purge deletes expired rows; truncation/rejection and history gating remove material independently of storage.
- Activation or effect: recovery text and context replacement are implemented, not complete continuity or successful recovery demonstrated.
- Evidence limits: source comments claiming a strict cap, latest numeric checkpoint or session precedence exceed some implementation details; see qualifications below.
SRC-1, equipa/checkpoints.py, equipa/sessions.py, equipa/loops.py, equipa/parsing.py, equipa/prompts.py; streaming/heartbeat wiring search in equipa/agent_runner.py, equipa/heartbeat.py.
Admission and transformation are computational, not per-snapshot human review. Hard checkpoints save available output on timeout/max turns; loader lexically sorts filenames, not parsed attempt integers. Soft checkpoint code keeps a 2000-character output prefix and path sets; streaming callers save periodically (interval ten turns) and on suspected compaction. Session capture combines soft state with .forge-state.json and twenty recent tool calls. Unknown original state-file authorship does not erase these known transformations. Session truncation cuts partial output, drops oldest tool entries, then halves output. Its 32-KB target is not an invariant when other fields alone exceed it: the loop can clear reasoning and break while other fields remain. Rendered resume prompts have a separate final byte-cut guard.
Session restore chooses latest nonexpired row by last_seen/id, inserts rendered context at history start, then loop independently loads a hard checkpoint and appends its compacted prompt. Contrary to the session module's precedence comment, this inspected loop can retain both. More importantly, _build_dev_extra_context omits regular history entirely without anti_compaction_state, including restored context; with that flag, multiple entries from cycle two onward are concatenated then limited to the first 400 words. This keeps the beginning, not necessarily the most recent failures. Paralysis warnings remain, and agent messages are prepended.
Compaction is an implemented deterministic derivation: code/log compression and deduplication, structured SUMMARY/FILES_CHANGED/BLOCKERS/DECISIONS/REFLECTION extraction, reject-mode sanitization, then first 200 words. If no structured material survives, sanitized output tail is the alternative; withheld sections do not fall back to their raw rejected text. These summaries feed subsequent invocations, hence are retained memory within a run even when not durable across process exit. Reason retention is conditional on extraction/truncation, not guaranteed.
sections = { "SUMMARY": _extract_section(raw_output, "SUMMARY"), "FILES_CHANGED": _extract_section(raw_output, "FILES_CHANGED", max_lines=-1), "BLOCKERS": _extract_section(raw_output, "BLOCKERS"), "DECISIONS": _extract_section(raw_output, "DECISIONS"), "REFLECTION": _extract_section(raw_output, "REFLECTION", max_lines=3), } ---
equipa/parsing.pyelse: extra_context = "\n\n".join(paralysis_entries) if paralysis_entries else "" ---
equipa/loops.py
The recovery wrapper instructs not to reread and to resume; hard-checkpoint prompt additionally asserts reading caused failure and demands immediate edits. That is authored guidance, not a diagnosis proved by a timeout. No correctness oracle admits these explanations. Rejection sanitizers block matched injection patterns, not false task interpretations. Rollback/recovery means retry from another retained source or project files; no reconstitution of the original attention state is established.
MEM-RTE-project-context
Label: Project context
Endpoints: operator/agent session entries → project tables → latest-note/unresolved-question/recent-decision selection → sanitized task prompt. Owner: instruction-driven interactive agent or external DB writers on write side, EQUIPA query/formatter on automatic read side. Distinct from supplied RT-RTE-dev-test. SRC-1, .claude/skills/forge-end/SKILL.md, equipa/tasks.py, equipa/prompts.py.
- implementation conclusion status: wired
- operation conclusion status: uninspected
- Immediate return: project-context dictionary or assembled task block; interactive skill requests confirmation of recorded session changes.
- Later read-back: later task construction automatically includes project history; interactive start/context/search instructions afford additional retrieval.
- Delegated visibility: latest summary/next steps, unresolved question/context and five recent decision/rationale/date entries; not full session history.
- Selection predicate: project ID, session date, question resolved flag and decision date; no topical decision search in this automatic path.
- Invalidation or expiry: newer session replaces older automatic visibility; resolution removes a question from automatic query; decision recency can evict an old entry without deletion.
- Activation or effect: project text delivery is wired; interactive skill execution, storage contents and actual reliance unobserved.
- Evidence limits: supplied skill requests human input but does not prove per-entry human approval for all DB writers; arbitrary direct SQL and external tool implementation are not this formatter's admission mechanism.
forge-end asks which tasks/decisions/blockers/summary to record and prefers sanitizing MCP tools with SQL fallbacks. Its summary fallback explicitly requires sanitize_session_note. Decision/question fallback SQL is not an enforced invocation of that sanitizer. Automatic task prompt construction treats these fields as agent-writable: reject-sanitizes, substitutes a withheld marker on rejection, and boundary-escapes blocks. Lesson default cap is 500 characters, session notes 50,000, decision context 8,000; large sanitized notes can therefore exceed overall prompt targets. Human contribution is afforded interactive supplying/editing, not universal item approval. Operating mode is session bookkeeping, not expected-answer evaluation. Withdrawal/editing by database state is possible; comprehensive decision supersession control is uninspected. Notably automatic decision query does not filter decision status, so “superseded” status alone is not established as withdrawal from this consumer.

- Later read-back: MCP client can request stored history in a later invocation; installed or actually exercised agent integration is uninspected.
- Delegated visibility: selected database fields returned by tool; downstream client incorporation and authority are external/uninspected, not equivalent to system-prompt assembly.
- Selection predicate: tool name plus clamped limit; lesson error_type LIKE filter; project/session queries use project selection where specified.
- Invalidation or expiry: query limit/date order controls returned subset; inspected lesson handler does not filter active state.
- Activation or effect: callable retrieval capability; no demonstrated subsequent model behavior.
- Evidence limits: registered handler does not establish deployed client consumption; read-time protections of automatic injection cannot be assumed for this separate raw-field response.
MCP lesson query can return a pruned lesson because both branches omit active = 1. It also returns error signatures and metadata beyond shortened task guidance. Thus SIMBA pruning is consumer-specific withdrawal, not complete access revocation. In the inspected registry search, the equipa_decision_add and equipa_session_note_add names recommended by the interactive skill were not found; their presumed server-side sanitation is not established here. Treat them as a documentation/integration ambiguity rather than fabricate implementation.

MEM-RTE-initiative-memory
Label: Initiative memory
Endpoints: sibling task output → ORCHESTRATOR_OUTPUT parse → sanitized summary/decision append to initiative plan → later task-ID/initiative-ID prompt load. Owner: orchestrator completion helper and plan renderer. Different from RT-OBJ-task-branch's product artifact and RT-RTE-dev-test's phase control. SRC-1, equipa/initiative.py, equipa/dispatch.py, equipa/prompts.py.
- implementation conclusion status: wired
- operation conclusion status: uninspected
- Immediate return: plan-update boolean or prompt section; append failures are best-effort, not task vetoes.
- Later read-back: a subsequent task carrying initiative_id loads the plan from project path and receives historical summary/decision text.
- Delegated visibility: shared plan contains selected task summaries and decision/rationale pairs, not original sibling conversations or tool histories.
- Selection predicate: task initiative_id and project directory, plan ensure/load and completion output parsing; missing output block produces placeholder.
- Invalidation or expiry: capped fields and first twenty decisions can omit content; plan edit/replacement changes future reads; no general plan-expiry policy established.
- Activation or effect: persisted cross-task text and automatic prompt delivery wired; task reliance and completeness unobserved.
- Evidence limits: full plan governance and cross-worktree consistency not audited; appended status labels are orchestrator summaries, not independent proof of task correctness.
Admission: agent proposes structured summary/decisions; software parser/sanitizer and field/count caps admit a bounded append; operator can supply original plan via a declared edit surface, with actual governance uninspected. No per-append human approval or semantic correctness oracle. Guidance requests summary and decisions/rationales for later siblings. Summary cap is 4096 characters; decision and rationale each 512; twenty decisions. Reasons can survive here as explicit rationale pairs, unlike the inspected SIMBA INSERT. File append uses locking and atomic rewrite; dispatch attempts commit to task branch. This does not make arbitrary plan content trusted instructions: prompt renderer wraps it under a random fence and says use historical data only. Operating mode is shared working memory for open long-horizon tasks. Recovery is placeholder or skipped append/logging, not reconstruction of missing reasons.

Claims
MEM-CLM-context-budget
Label: Context budget
Claimed operation: build_system_prompt describes an 8K target, reductions and priority trimming. Implementation estimates tokens by four characters each, uses a 10K aggressive-trim threshold, leaves static prefix/task content protected and logs OVER rather than refusing an oversized result. Therefore this is best-effort selection, not a hard context-window invariant. SRC-1, equipa/prompts.py, equipa/parsing.py. Closest supplied finding is RT-RTE-dev-test's context account; this is the narrower budget claim, not a duplicate route.

MEM-BAP-experience-context
Label: Experience context
Consumer: task inference invocation. Channel: assembled dynamic suffix containing historical lessons/episodes/project and initiative context or reconstructed prior output. Force: model-interpreted historical data/advice under untrusted wrappers, not executable veto or assured compliance. Horizon: current call; selected retained objects can recur across later calls/tasks/sessions. Different from RT-BAP-role-instructions's standing instruction body; selection code separately has executable authority over inclusion. SRC-1, equipa/prompts.py, equipa/lessons.py.

Annotations
On RT-RTE-dev-test
Memory-specific frame crossing: SQLite agent messages select newest bounded unread records for the destination task/role, then reverse them to chronological display. Formatting limits scanning/text and elapsed sanitation time; loop marks messages read after formatting, and mark_messages_read consumes all unread matching-role records, including older entries omitted by the read cap. This can permanently suppress omitted content from the ordinary unread route without deleting raw rows. See MEM-OBJ-agent-messages. SRC-1, equipa/messages.py, equipa/loops.py.
"""UPDATE agent_messages SET read_by_cycle = ? WHERE task_id = ? AND to_role = ? AND read_by_cycle IS NULL""", ---
equipa/messages.py
Separate role invocations resemble temporary frames: selected prompt inputs enter; worktree changes, result text, messages and summaries escape; private full conversation does not. Unlike a call stack with dereferenceable pointers, a summary in the next prompt is text by value; file/path metadata does not automatically recover contents. Disk offload would supply a reload pointer, but its ordinary activation is not established. This annotation uses the supplied runtime's developer/tester isolation evidence rather than claiming a second independent trace.
On RT-OBJ-role-prompts
Material editing/adoption surfaces include baseline file replacement/appends and versioned GEPA bodies; static prefix combines common text, selected role and standing orders. These are retained natural-language instructions. The dynamic suffix additionally delivers learned experience with a different historical-data authority. The inspected selector reads selected version bytes, not optimizer rationale. Do not merge instruction artifacts with SQLite episode prose or dense model weights. SRC-1, equipa/prompts.py; supplied RT-RTE-gepa-prompts supplies admission/rollback evidence.
On RT-RTE-rlm-review
Working-context control, from supplied evidence: code retains repository text in REPL state outside outer model input; outer successor calls replace prior conversation with decomposition prompt and latest block results, while aggregate return retains more. Focused subquery returns cross isolated frames; no durable later-session read-back is established. This is current-call reachability/eviction, not evidence of learned persistent review capability.
On RT-RTE-ollama-tools
Working-context control, from supplied evidence: oversized tool results lose their tails before model consumption; messages otherwise accumulate within the call and are discarded on return. Filesystem product state can persist separately. No source-grounded reload of truncated tails or full durable conversation is established by this route.
Write side
The source supports several distinct raw-to-derived chains, not one universal memory writer:
- Task result text → parsed sanitized episode/reflection → SQLite; outcome/exposure → Q updates → later ranking. Separate reflection invocation sees only a tail and may supply retrospective interpretation. The raw run is evidence for derivation, not itself the learned rule. See MEM-RTE-episode-feedback.
- Repeated failure trace → template lesson; reflected mixed-outcome episodes → model-proposed SIMBA rules → structural/duplicate gate → stored text → active injection. SIMBA effectiveness/pruning updates future eligibility, with explicit-query bypass retained. Human launch is not per-rule approval. See MEM-RTE-lesson-maintenance.
- Streaming output → hard output checkpoint or sampled soft state; sampled state + forge-state + tool metadata → session; output → deterministic 200-word summary → filtered next-call history. These transforms are software curation even if no model “summary” call occurs. They do not preserve every reason. See MEM-RTE-context-recovery.
- Human/agent session entries → notes/questions/decisions; sibling output → initiative plan with decision/rationale pairs. Known derived transforms coexist with unresolved original file/database authorship. See MEM-RTE-project-context, MEM-RTE-initiative-memory.
- Run evidence → file/config/GEPA/autoresearch proposals → route-specific admission and later loader. These are already established in the supplied runtime records; no new identity is allocated here. A scoring proxy and installed prompt are not demonstrated future capacity improvements.
Reject-mode sanitizer is an actual string gate: pattern match returns empty; accepted text has markup neutralized and length caps. It does not prove factual truth or complete injection resistance. Rejection at one point does not erase raw source output or guarantee another tool's query is sanitized. SRC-1, lesson_sanitizer.py.

Read-back
Automatic push is initiated by the orchestrator assembling another invocation, not by an independent agent requesting particular memories: task metadata selects experience, project ID selects recent project records, initiative ID selects shared plan and task/role selects recovery. The model receives formatted copies, not live references to tables. Request-driven MCP queries are separate: a client names a tool/filter, the server returns records, and downstream incorporation remains uninspected. Static shipped prompts are context inputs, not read-back through use; their accumulated edits/version selections qualify separately under supplied evolution routes.
Prompt budget is selective and approximate. Static prefix >6000 estimated tokens lowers episodes from three to two. Above 8000 estimated total, episode section gets a 500-token-equivalent character cap; above 10000, lessons get 300 and prior-work summary 400. _trim_prompt_section cuts at characters until another heading/separator, not semantic importance, and does not prove wrapper closure. Later oversized output is logged, not refused. Counts/Q tracking are recorded before these trims. Thus “times injected” is not proof complete material reached inference. See MEM-CLM-context-budget.
LITM reordering in equipa/prompts.py acts only when ## Prior Work Summary appears, ordering most recent, middle, oldest sections; its alpha/beta/gamma arguments are not used in that ordering body. Consolidation changes the outer heading to ## Previous Attempts, which can defeat that matching route. The audit module can write litm_weights configuration, but scoped search found references only in forgesmith_litm.py, not an EQUIPA config-weight reader; the inspected call uses default arguments. Retained adjustment is not established to reach this consumer. This is a named integration gap, not a system-wide absence claim.
Optional disk offload requires agent_id and session_dir before compaction; it keeps full compressible oversized output with preview/path. The inspected build_compaction_summary and build_checkpoint_context calls supply neither. The helper is a storage/return capability, not a demonstrated later consumer route. The foreground agent could potentially read an accessible file path, but no actual reload trigger/caller is established. Provider/Claude host compaction remains opaque and is not attributed to EQUIPA's own deterministic compactor.
Integration issues
- Qualify session precedence and guaranteed delivery. MEM-RTE-context-recovery supplements RT-RTE-dev-test: sessions and hard checkpoints both enter history; ordinary history is omitted without anti_compaction_state. Session module's “wins” statement is not an unconditional implemented priority. Consequence: cannot claim full restoration merely from a loaded session.
- Qualify strict storage/budget and latest-checkpoint labels. MEM-RTE-context-recovery, MEM-CLM-context-budget: lexical filename selection is not general numeric latest; state truncation may exceed target if other fields dominate; prompt hard-limit constant does not enforce hard refusal. No supplied runtime guarantee is withdrawn; these refine source labels.
- Exposure is approximate. MEM-OBJ-q-value, MEM-RTE-experience-retrieval: task-keyed tracker overwrites and pre-trim counters prevent full causal exposure attribution. Q reward updates do not demonstrate an injected episode was useful.
- SIMBA source-name and graph identity questions. MEM-RTE-lesson-maintenance, MEM-RTE-experience-retrieval, MEM-OBJ-lesson-graph-edges: generated source label misses scoring helper filter; separate lesson/episode IDs share untyped graph edges. Consequence: preserve known active lesson delivery while withholding a coherent rule-bonus/semantic-graph claim. Flag-enabled graph failures also skip final score sort.
- Consumer-specific withdrawal and alternate admission. MEM-RTE-memory-queries can expose inactive lessons and lacks the automatic injection's formatting gate. Interactive skill recommends write tools absent from inspected registry search. Consequence: neither universal withdrawal nor server-side sanitation for those named write tools is established.
- Optional embedding and LITM propagation gaps. MEM-RTE-episode-feedback shows ordinary telemetry does not forward config enabling record-time episode vectors; audit-written LITM config has no established reader in inspected ordering path. Backfills/alternate callers remain uninspected, so vector or weight adoption is not globally denied.
- Part/container and possible overlap. MEM-RTE-context-recovery is explicitly a part of RT-RTE-dev-test, not its replacement. Other new records compare with nearest supplied records in their declarations. No possible duplicate referent remains asserted: static role/evolution routes are annotated or cited, not redeclared. Initiative plan differs from its branch container; embedding inference operation differs from chat inference even if deployment service is shared.
- Remaining included internals. Coverage table retains uninspected plugin writes, detailed scorer/OPRO/training/evaluation/rollback, setup/migration/sync and scheduling. Their missing admission lineage/consumer facts prevent complete improvement-plane, training or cleanup assurance. Existing runtime positives stand; uncertainty is not an absence finding. No cross-system comparison or adoption recommendation follows.
Limitations and checks
This is source analysis, not observed operation or a causal experiment. Implemented transformation and later prompt delivery do not establish reliable interpretation, truthful retained reasons, task correctness or improved capacity. The primary direct evidence is frozen implementation; interactive skill procedures remain instructions. Unresolved and available-but-uninspected parts are named above rather than collapsed into the inspected episode route.
Method identity is the supplied memory job, member type, worker rules and records contract; no independent methodology revision is inferred. Source identity and full HEAD were rechecked, with clean target status. Only authorized candidate/answer/identity paths were written. Deterministic validation: the invocation's prepared commonplace-run validate-output check passed after disambiguating the sanitizer quotation attribution. This establishes structural/quotation validity, not semantic completeness or job acceptance.
